Initial Enquiry & Assessment
We start by listening. Our team discusses your needs, goals, and funding, then arranges an assessment to understand exactly what support will make the biggest difference in your life.
What happens in your first conversation
- we ask about your needs, your goals and what a good day looks like for you
- we ask who else is involved, such as family, carers or a support coordinator
- we talk about how your care could be funded, and explain your funding options
- we arrange an assessment at a time that suits you
What the assessment covers
The assessment helps us understand your health, your daily routine, your home and the support that would make the biggest difference. It is a conversation, not a test, and you are welcome to have a family member or support person join you.
What is useful to have ready
- details of any current supports or plans, such as an NDIS plan or a Support at Home approval
- a list of the health conditions and medications that are relevant to your care
- any questions you would like to ask us
Initial enquiry: questions
Do I have to decide everything at the assessment?
No. The assessment is where we listen and explain. You decide what happens next, and the next step is your personalised care plan.
