Medication management

STA Community Nursing nurse sorting tablets into a weekly pill organiser for medication management at home

Our nurses provide full clinical oversight of your medications — from dose reminders and administration to reviewing your medication schedule for safety. We help reduce the risk of missed doses or interactions, particularly for people managing multiple prescriptions, and liaise with your GP or pharmacist when changes are needed.

How our nurses can help

  • dose reminders, so nothing is missed
  • giving medicines as prescribed
  • reviewing your medication schedule for safety
  • helping reduce the risk of missed doses or interactions
  • talking to your GP or pharmacist when a change is needed

Reviewing your medicines

If you take several medicines, ask your GP whether a Home Medicines Review, where a pharmacist visits your home to go through what you take, could be right for you. Our nurses can also help you prepare your questions.

If a mistake happens

If you think someone has taken the wrong medicine or the wrong dose, call the Poisons Information Centre on 13 11 26, any time of day (see healthdirect). In an emergency, call 000.

Medication management: questions

Can your nurses change my medicines?

No. Only your doctor can change your prescription. If something worries our nurses, they will contact your GP or pharmacist.

Is this the same as giving injections?

Injections are part of our injection assistance service. Medication management covers the wider picture of what you take and when.
