Preparing for Hospital Discharge

Tips for a smooth transition home with the right support in place. Leaving hospital can feel rushed, especially after a stay you did not plan for. A little preparation makes the first few days at home much easier.

Before you leave hospital

Hospital staff usually run a discharge planning process before you go home. It helps to ask:

  • what follow-up appointments are needed, and with whom
  • what your new or changed medications are, and how to take them
  • whether you will need wound care, help with mobility, or other clinical support at home
  • who to contact if something does not seem right after you get home

The healthdirect guide to going home from hospital has a general checklist you can work through with hospital staff.

Arranging support before you go home

Where possible, arrange your support before discharge day rather than after. That can mean a nurse to manage medications and wounds, a support worker to help with everyday tasks while you recover, or transport to your follow-up appointments. If you already have a plan through the NDIS or Support at Home, tell the hospital social worker, who can help coordinate things on your end.

Making your home safe to return to

Before you arrive home, it helps to clear walkways of rugs and clutter, make sure frequently used items are within easy reach, and check that any equipment you have been prescribed, such as a walker or shower chair, is set up and ready. Our occupational therapists can assess your home and suggest changes if you are recovering from a fall, surgery or a new diagnosis.

The first few days at home

Go gently. It is common to tire more easily than expected. Keep a note of your new medications and any symptoms that worry you, and do not wait for your next scheduled appointment if something feels wrong. Our nursing team can visit at home to check in during this period, and our respite care can support family carers who are also adjusting.
